Marikina Passport on Wheels

Mobile passporting services will be held on March 9, 2018 at the Marikina City Hall Quadrangle. This is in partnership of the local city government of Marikina and the Department of Foreign Affairs.  You can call 646-0373 for more details.





Rquirements for 1st Time Applicant:
– Authenticated Birth Certificate – PSA Copy
– At least 2 proof of identity / supporting documents indicating full name, date and place of birth and citizenship
– Voters Affidavit (if late registered)

For Female
– PSA Copy Marriage Contract – if Married
– PSA Copy Death Certificate – if Widow

For Annulled/Divorced Aplicant
– Certified True Copy Divorce Paper – if Divorce
– Certified True Copy Annulment – if Annulled

For Minor (below 18 years old)
– Personal appearnce of mother

For minor is Not Traveling with parent
– Original & photo copy DSWD Clearance
– Affidavit of consent

For minor both parents are abroad
– Affidavit of consent (authenticated Phil Embassy)
– Special Power of Attorney (authenticated Phil Embassy)
– Passport of traveling companion
– Identification card of parents & traveling companion

FEE: Php 1,200.00 + Php 150.00 mailing fee




Requirements for Passport Renewal:
– Old Passport (Original)
– Authenticated Birth Certificate – PSA Copy
– Authenticated Marriage Certificate – PSA Copy – if Married Female
– Xerox copy 1st & last page of old passport
– Xerox copy of Latest Arrival & Departure (if any)
– Any Document or ID with Complete Middle Name

FEES: Php 1,200.00 + Php 150.00 MAILING FEE

For Passports damaged by Typhoon ONDOY:
Intact but Damaged:
– Affidavit of mutilation,
– damaged passport,
– requirements as NEW applicants

FEES: Php 350.00 (Damaged) + Php 1,200.00 + Php 150.00 MAILING FEE

Lost Expired Passports:
– Affidavit of loss
– requirements AS NEW applicants

FEES: Php 350.00 (LOST) + Php 1,200.00 + Php 150.00 MAILING FEE

– Unified Multi-purpose Identification Card (UMID) Card
– Land Transportation Office (LTO) Drivers License
– Professional Regulatory Commission (PRC) ID
– Overseas Workers Welfare Administration (OWWA)
– iDOLE Card
– Philippine National Police (PNP) Firearms License
– Senior Citizen ID
– Person with Disabilities (PWD) ID
– School ID (for student)
– Digitized BIR ID
– Current Valid ePassport
– Integrated Bar of the Philippines (IBP) ID
– Transcript of Records (TOR)

List of Supporting Documents (at least 3)
(Issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant)
– Baptismal
– Dedication from church
– Commission on Election (Commelec) Voter’s ID
– Commission on Election (Commelec) Voter’s Registration
– NBI Clearance
– Police Clearance
– Barangay Clearance
– Elementary / High School form 137 with dry seal
– Lant title
– Seaman’s Book
– Governemtn Service Record
– SSS-E1 form or Microfilmed Copy of SSS E1 Form
– Income Tax Return
– School Year Book
– Community Tax Certificate
